Recurrent Dislocation of the Patella in Patient with Total Knee Arthroplasty: A Case Report

Abstract

Introduction: Most of the complications related to the kneeextensor mechanism in a total knee arthroplasty are caused by patellofemoral instability. The aim of this article is to illustrate how challenging it might be to treat a patient with recurrent dislocation of the patella after total knee arthroplasty (TKA).

Case Report: A 70-year-old female patient was followed by the Physical Medicine and Rehabilitation Department after right TKA status (bi-compartmental) complicated by recurrent dislocation of the patella. After falling once, she suffered a proximal fracture of the tibia with detachment of the tibial component, having undergone surgical revision. After this event, the patient was operated four more times for recurrent dislocation of the patella and dislocation of the prosthesis itself.

Discussion: The etiology of patellar instability after TKA is often multifactorial, with the main cause being surgical technique errors that may lead to poor positioning of the prosthetic components. Most treatment options combine a surgical review and a targeted rehabilitation plan.

Conclusion: Patellofemoral instability is a relevant complication in patients submitted to TKA, and may be avoided by multiple surgical and non-surgical strategies.
